Fall 2008 Player Registration Form
Please complete this form and bring to tryouts to save time!
Also bring payment of $125 to Northampton Soccer Club,
and a parent or guardian to sign in.
Full or partial scholarships are available.

Plenty of water,
soccer cleats,
protective shinguards,
and your own soccer ball.

All new players entering U11 and above,
along with all returning players entering U11 or U13:
Bring a 1x1 yearbook-style photo.

U9 and U10 Boys:
Please wear a WHITE shirt

U9	Born on or after August 1, 1999
U10	Born on or after August 1, 1998	... or younger and entering Grade 4
U11	Born on or after August 1, 1997	... or younger and entering Grade 5
U12	Born on or after August 1, 1996	... or younger and entering Grade 6
U13	Born on or after August 1, 1995	... or younger and entering Grade 7
U14	Born on or after August 1, 1994	... or younger and entering Grade 8
